• remediation costs imposed by regulators,
• pollution that is accidental and rapid or gradual,
• first party business interruption and emergency mitigation costs.

The policy is unable to cover identified / known pollution exposures (perhaps discovered in M&A due diligence) or first party liability, i.e. digging up your own land and the resulting clean up costs.
